射频消融术对原发性肝癌患者甲胎蛋白和Th1、Th2细胞因子的影响

目的:观察原发性肝癌患者经射频消融(RFA)治疗后外周血甲胎蛋白(AFP)和Th1、Th2细胞因子的变化,探讨射频消融术对原发性肝癌患者治疗效果与免疫功能间的关系。方法:应用射频消融治疗仪治疗原发性肝癌46例,采用化学发光方法检测AFP值,酶联免疫吸附(ELISA)方法检测患者在射频消融前和治疗后14天、28天外周血Th1细胞因子白介素(IL)-2、肿瘤坏死因子(TNF-α)、干扰素(IFN)-γ]及Th2细胞因子(IL-4、IL-6、IL-10)的表达。结果:原发性肝癌患者射频消融治疗前外周血AFP值明显高于健康志愿者(P<0.05),Th1细胞因子降低,Th2细胞因子升高,与正常对照组相比,差异有统计学意义(P<0.05)。射频消融术后,大部分患者外周血AFP值明显降低,Th1细胞因子升高,Th2细胞因子降低,与正常对照组相比,差异有统计学意义(P<0.05)。结论:Th1、Th2细胞因子的水平状态在原发性肝癌射频消融治疗后与AFP的变化存在相关性,可能对肝癌的预后有一定指导意义。

Effect of radiofrequency ablation on AFP and Th1,Th2 cytokines of patients with primary hepatocellular carcinoma

Objective：To observe the change of the level of AFP and Th1,Th2 cytokines in PeriPheral blood of Pa-tients with Primary hePatocellular carcinoma（PeC）by radiofrequency ablation（RFA）. Methods：Chemiluminescence were used to detect AFP and ELISA were used to detect the level of serum IL - 2,IL - 4,IL - 6,IL - 10 IFN - γ and TNF - α in 46 cases with PeC using RFA before one day and after oPeration fourteen days and twenty - eight days. IL- 2,IFN - γ and TNF - α were used to rePresent cytokines of Th1 tyPe,and IL - 4,IL - 6 and IL - 10 to rePresent cytokines of Th2 tyPe. Results：Before RFA with Patients of PeC,AFP values in the serum was significantly higher than that in the healthy volunteer（P ﹤ 0. 05）,the level of Th1 cytokines was significantly lower,the level of Th2 cyto-kines significantly higher than those of the healthy control grouP,resPectively（P ﹤ 0. 05）. After RFA oPeration,AFP values in the serum was significantly lower,the level of Th1 cytokines were significantly higer,the level of Th2 cyto-kines was significantly lower than those of the healthy control grouP,resPectively（P ﹤ 0. 05）. Conclusion：The level of Th1,Th2 cytokines were correlated with the change of AFP in Patients of PeC on RFA,might have an imPortant guid-ing significance for the Prognosis of PeC.
