胃癌全胃切除术后早期肠内营养的临床观察

目的观察胃癌全胃切除术后早期肠内营养(EN)的效果。方法将36例行全胃切除术的胃癌患者随机分为EN组和肠外营养组(PN组)各18例。EN组采用液囊空肠导管给予肠内营养支持,PN组给予常规的营养支持。比较2组间手术前后体质量、白蛋白、前白蛋白、血红蛋白、外周血淋巴细胞、转铁蛋白水平,并观察肛门排气、排便时间及住院时间。结果 2组间手术前后体质量、白蛋白、前白蛋白、血红蛋白、外周血淋巴细胞、转铁蛋白水平比较差异无统计学意义(P>0.05)。EN组肛门排气时间、排便时间及住院时间均短于PN组,差异均有统计学意义(P<0.05)。结论 EN用于胃癌全胃切除术后,可早期促进肠道功能恢复,值得临床推广应用。

Clinical observation of early enteral nutrition for gastric cancer after total gastrectomy

Objective To observe the effect of early enteral nutrition（EN） for gastric cancer after total gastrectomy. Methods 36 cases patients with gastric cancer after total gastrectomy were randomly divided into EN group and parenteral nutrition group（ PN group） ,each of 18 cases. EN group was given enteral nutrition,PN group was given parenteral nutrition. Compared the weight,albumin,prealbumin,hemoglobin,peripheral blood lymphocytes and transferrin level before and after the operation of two groups. The exhaust time,defecation time and the time in hospital were observed. Results Before and after operation there were no significant difference of weight, albumin, prealbumin, hemoglobin, peripheral blood iymphocytes and transfen-in in two groups（ P 〉 0.05 ）. The exhaust time, defecation time and hospitalization time in EN group were shorter than those in PN group, the difference were statistically significant （ P 〈 0.05 ）. Conclusion EN for gastric cancer patient after total gastrectomy, can promote intestinal function recovery in early stage, worthy of clinical application.
