新医改下浦东新区医疗机构服务效率调查分析

目的对新医改实施以来浦东新区医疗机构的服务效率展开分析和评价,为制定和完善相关卫生政策提供参考。方法对浦东新区各级各类医疗机构的运行效率进行描述性统计,并通过横向比较和纵向趋势分析寻找其中的规律和特点。结果医院和社区卫生服务中心医师日均担负诊疗人次增加,妇幼保健院医师日均担负床日明显下降;次均门诊和住院费用逐年增长,但药费占比呈下降趋势;除民办医院外,各类型医疗机构病床使用率均在85%以上,妇幼保健院病床处于超负荷运转;社区卫生服务中心患者平均住院日超过25天。结论在推动新医改、提高医疗机构服务效率方面,浦东新区已经取得了一定的成效,但在控制医疗费用、调整和完善医疗资源配置结构、理顺各级医疗机构分工协作机制和提高医疗资源利用效率方面还有改进的空间。

Survey on Service Efficiency of Medical Institutions of Pudong New Area Under the New Medical Reform

OBJECTIVE To conduct analysis and evaluation on the efficiency of medical institutions in Pudong New Area since the latest health sector reform,and thus provide references for decision making process and its improvement.METHODS Describe the operational efficiency of different classes or types of medical institutions in Pudong New Area using statistics such as mean,proportion and rate,and identify the rules and characteristics through cross-sectional and for-and-aft comparisons.RESULTS The daily burden of out-patient attendances of physicians in hospitals and community health centers increased,while the daily in-patient care of maternal and child hospitals declined significantly;the average cost of out-patient visits and in-patient care increased year by year,but the proportion of expenditure on drug was decreasing;the bed occupancy was above 85 percent except for the private-owned hospitals,and the maternal and health hospitals were overladen;the average length of hospital stay in community health centers were above 25.CONCLUSIONS Pudong New Area has made relatively good performance in terms of promoting the latest health sector reform and enhancing the efficiency of medical institutions,however,there is still room for aspects like adjusting and improving the structure of medical resources allocation,optimizing the mechanism of division of labor and cooperation,and enhancing the efficiency of medical resources.
