不同治疗方法对糖尿病下肢震动感觉阈值的影响

目的：观察胰岛素治疗和非胰岛素治疗的2型糖尿病患者下肢震动感觉阈值（VPT）的差异,探讨血糖、血脂、糖化血红蛋白等因素对下肢VPT的影响。方法：选取糖尿病患者87例（胰岛素治疗组41例,口服药物治疗组46例）,正常对照组18例,检测FBG、TG、CHO、HDL、LDL、HbA1 c等生化指标,并进行双足VPT测定,比较各组间的差异,用直线相关方法分析双足VPT与各检测指标间的关系。结果：①糖尿病组FBG、HDL较对照组明显升高（P〈0.05）,糖尿病胰岛素治疗组CHO值较口服药物治疗组明显降低（P〈0.05）;②两糖尿病组双下肢VPT较对照组明显升高（P〈0.05）;③双下肢VPT与年龄、HbA1 c、糖尿病病程等成正相关关系（P〈0.05）。结论：高龄、血糖控制差、病程长等因素是糖尿病周围神经病变的主要影响因素,胰岛素治疗可能延缓糖尿病周围神经病变的发生发展。

Study on the effect of different medical treatment on lower limb vi bration perception threshold for diabetics

Objective：to observe the difference of lower limb vibration perception threshold in diabetics accepting insulin treatment and taking oral drugs,approaching the effects of some biochemical factors such as blood glucose,blood lipid,HbA1c on VPT.Methods：Plasma concentrations of FBG、TG、CHO、HDL、LDL、HbA1c were measured in 87 diabetics（41accepting insulin treatment,46 taking oral drugs）and 18 normal control,VPT was also performed.The difference between the three groups was compared.Results： ①FBG and HDL in diabetics were higher than in normal control（P〈0.05）.The serum level of HDL was lower in diabetics accepting insulin than diabetics taking oral drugs（P〈0.05）;② For diabetics,the VPT was obviously higher than normal control（P〈0.05）;③There was positive correlation between age,HbA1c,pathogenesis and VPT（P〈0.05）.Conclusion：Advanced age,bad glucose controlling and long pathogenesis may be the main factors of diabetic peripheral neuropathy.And insulin therapy may delay the onset and development of diabetic peripheral neuropathy.
