摘    要:目的探讨脉冲振荡肺功能与食管癌贲门癌术后呼吸系统并发症的关系。方法对937例食管癌贲门癌患者术前行脉冲振荡肺功能检测,比较各项指标对术后并发症的影响,并与常规肺通气功能比较。结果有并发症组和无并发症组在共振频率、呼吸总阻抗、周边气道总阻力上有明显差异(P<0.01),呼吸衰竭组共振频率明显高于其他并发症组(P<0.01),脉冲振荡肺功能与常规肺功能各指标间呈负相关关系。结论脉冲振荡肺功能测定有助于预测食管癌贲门癌术后呼吸系统并发症。

Abstract:Objective To evaluate the relationship between the pulmonary function with impulse oscillation system and postoperative respiratory complications of esophagus carcinoma and gastric cardiac carcinoma.Methods The preoperative pulmonary function of 937 cases including 582 esophagus carcinoma and 355 gastric cardiac carcinoma were measured by an impulse oscillation technique for the difference between the patients with or without respiratory complications,and compared with conventional pulmonary ventilation function.Results There were distinct differences between the patients with or without respiratory complication on resonant frequency(Fres),respiratory impedance(Zrs),peripheral airway resistance(R5-R20)(P<0.01).The patients with respiratory failure was higher than the others on Fres(P<0.01).Fres,Zrs,R5-R20 were negatively correlated with the indexes of pulmonary ventilation.Conclusion The pulmonary function detected with impulse oscillation system is helpful to forecast the postoperative respiratory complications after surgery of esophagus carcinoma and gastric cardiac carcinoma.
