| 摘 要： | 目的 探讨门冬氨酸钾镁(PMA)在弥漫性轴突损伤(DAI)中的保护作用.方法 选择符合DAI诊断标准的患者46例,随机分为常规组和PMA组,PMA组在常规组给予吸氧、脱水、器官功能支持、营养神经及预防并发症等综合治疗的基础上加用PMA治疗.于入院时、治疗后3 d、7 d、14 d分别检测血清NSE和MBP的浓度变化,并于入院时及入院1周和2周进行格拉斯哥昏迷评分(GCS)、3个月后进行格拉斯哥预后分级评分(GOS),以观察远期治疗效果.以同期20例健康体检者作为对照组.结果 DAI患者血清NES和MBP的浓度均显著高于健康人(P<0.05).治疗后7 d、14 d PMA组血清NSE和MBP浓度分别均低于常规组,差异均有统计学意义(P<0.05).PMA组在入院时GCS与常规组比较,差异无统计学意义(P>0.05),但入院1周和2周的GCS及治疗3个月后的GOS均显著高于常规组,差异有统计学意义(P<0.05).结论 PMA能降低DAI患者血清NSE、MBP含量,并能明显改善患者的预后.
