应用Maruyama软件术前预测胃癌淋巴结转移状况的可行性研究

目的 研究应用Maruyama软件术前预测胃癌淋巴结转移范围的可靠性与可行性。方法 分别对246例和134例胃癌患进行回顾性和前瞻性研究。将患性别、年龄、肿瘤Borrmann分型、组织学类型、浸润深度、部位、大小等预后因素输入计算机，应用Maruyama软件对淋巴结转移范围进行分析，再将其结果与术后病理诊断进行对比研究。结果 本组胃癌淋巴结转移判断的正确率较高。在回顾性研究中，N1为89．0％、N2为87．4％、N3为95．5％，总体达89．0％；在前瞻性研究中，上述各组淋巴结转移的术前预测正确率分别达87．3％、82．1％、88．8％和87．3％。结论 通过Maruyama软件分析，术前预测胃癌淋巴 结转移可达较高正确率，但为进一步提高其临床应用价值，还应改善术前判断肿瘤浸润深度的可靠性。

Study of preoperative assessment of lymphatic metastasis in patients with gastric cancer by Maruyama computer program

Objective To evaluate the reliability and the f easibility of the application of Maruyama computer program on the preoperativ e assessment of lymph node metastasis in patients with gastric cancer.Medthods Computer aided preoperative predic tions of lymph node metastasis were d etermined by using the prognostic factors of sex,age,Borrmann classification,histology,depth of wall penetration,location an d diameter of the tumor.Data were analysed in 246cases for retrospective s tudy and in 134cases for prospective study respectively.The predicated results were compared w ith the postoperative pathological findings.Results The prediction of lymph node metasta sis was highly accurate with N 1 89.0%,N 2 87.4%,N 3 95.5%and totally 89.0%in the retrospective study;N 1 87.3%,N 2 82.1%,N 3 88.8%and totally 87.3%in the prospective stu dy.Conclusions The Maruyama computer program may pr ovide preoperative information of lymph n ode metastasis in patients with gastric cancer.In order to exert more practical value of this computer pro gram,it is necessary to increase the accuracy of judging the tumor invasive depth before surgery.
