| 摘 要： | 目的对比观察前交叉韧带（ACL）损伤的MRI表现与关节镜所见；试对前交叉韧带的损伤进行MRI分级和对磁共振前交叉韧带扫描方法的改进。方法对217例前交叉韧带进行关节镜与MRI对照观察。方法MRI诊断符合168例（77．4％），对不全断裂、完全断裂的诊断不足44例（30．6％），符合率分别为78．6％和65．7％；假阴性17例（7．8％）。髁间棘撕脱骨折伴ACL损伤9例（4．4％）；ACL损伤伴内侧副韧带损伤42例、伴后交叉韧带损伤25例。方法根据前交叉韧带损伤的程度及表现，MRI将损伤分为四级。本组MRI对ACL完全撕裂的损伤诊断不足，分析原因为ACL解剖特点导致MRI在显示上存在难点，以及ACL损伤后的一些病理改变导致MRI显示为假阴性或诊断不足。改进扫描技术、观察ACL损伤的间接征象，并结合物理检查才能提高MRI诊断符合率。
