不同剂量大黄附子细辛汤对慢性肾衰竭患者中医证候积分及肾功能指标的影响

目的探讨不同剂量大黄附子细辛汤对慢性肾衰竭（cRF）患者中医证候积分及肾功能指标的影响。方法将早中期CRF患者85例随机分为A、B2组。A组（n=43）给予小剂量大黄附子细辛汤，B组（n=42）给予大剂量大黄附子细辛汤，比较2组患者中医证候积分、肾功能、电解质水平，以及不良反应。结果2组患者的中医证候积分均较治疗前显著下降，但B组下降幅度更为显著。B组治疗后血肌酐（Scr）、尿酸（UA）及内生肌酐清除率（Ccr）水平均较治疗前显著改善，且改善幅度明显大于A组。2组不良反应无显著差异。结论大剂量的大黄附子细辛汤对于CRF症状及肾功能的改善程度优于小剂量，且不增加不良反应的发生率及程度，值得临床进一步研究。

Influence of different doses of Dahuang Fuzi Xixin Decoction on TCM symptom scoring and renal function of patients with chronic renal failure

Objective To explore the different doses of Dahuang Fuzi Xixin Decoction on TCM symptom scoring and renal function of patients with chronic renal failure（CRF）. Methods Eighty- five CRF patients were divided into two groups randomly. Group A （n = 43） received small doses of Dahuang Fuzi Xixin Decoction, while group B received large doses. The TCM symp- tom scoring, chronic renal failure, electrolyte, and adverse reactions were compared. Results Af- ter treatment, the TCM symptom scorings in the two groups decreased noticeably, and the decreas- ing degree in group B was larger than that in group A. After treatment, the SCr, UA and CCr in group B improved obviously, and the improving degree was larger than that in group A. The ad- verse reactions between the two groups showed no obvious difference. Conclusion Large doses of Dahuang Fuzi Xixin Decoction has better effect on symptoms and renal function of CRF patients, and does not increase the incidence and degree of adverse reactions. It＇s worthy of further research.
