糖尿病伴发缺血性脑血管病患者肾动脉狭窄情况的临床研究

摘    要:目的初步探讨糖尿病(DM)伴发缺血性脑血管病患者动脉粥样硬化性肾动脉狭窄(ARAS)的检出率并与非DM脑血管病ARAS发生的情况进行比较。方法总结分析167例因缺血性脑血管病而行全脑血管造影并同时进行非选择肾动脉造影患者的临床资料,其中DM38例,非DM129例,比较DM和非DM脑血管病ARAS的检出率,对ARAS的发生与脑血管病的严重程度进行相关性分析。结果167例患者共发现ARAS63例(37.72%),其中DM组为17例(44.74%),非DIM组为46例(35.66%),2组比较无统计学差异。但脑动脉狭窄患者中ARAS的检出率为42.64%(55/129)显著高于非脑动脉狭窄患者(P〈0.05)。结论ARAS的发生与脑血管病有一定相关性。

Abstract:Objective To study detection rate of atherosclerotic renal artery stenosis (ARAS) in patients with diabetes mellitus (DIM) accompanied with ischemic cerebrovascular dis- ease, and compare it with the situation of non-DM cerebrovascular disease ARAS. Methods The clinical data of 167 patients with cerebral angiography and non-selective renal artery angiography were retrospectively analyzed because of ischemic cerebrovascular disease, including 38 cases with DM, 129 cases with non-DM. Detection rates of ARAS between DM and non-DM cerebrovascular disease were compared. Correlation analysis between ARAS incidence and severity of cerebral vascu- lar disease were conducted. Results The research found that there were 63 cases (37.72 % ) with atherosclerotic renal artery stenosis in 167 cases, 17 cases (44.74%) in DM group and 46 cases (35.66%) in DM group, there being no statistical difference in two groups. But cerebral artery stenosis in patients with ARAS positive rate was 42.64 % (55/129) which was significantly higher than that in patients with cerebral arterial stenosis (P 〈 0.05). Conclusion There is a certain cor- relation with the occurrence of ARAS and cerebrovascular diseases.
