不同手术方法治疗老年粗隆间骨折疗效对比分析

目的比较分析滑动加压鹅头钉（DHS）和改良股骨近端加锁髓内钉（PFNA）治疗59例老年股骨粗隆间骨折的临床疗效。方法对59例老年股骨粗隆间骨折行手术治疗，其中DHS固定31例，PFNA固定28例。比较两组术中和术后多项相关指标并行统计学分析。结果比较两组的手术时间、术中失血量和术后引流量、负重行走时间差异有统计学意义；而术后髋关节评分差异无统计学意义。结论DHS和PFNA治疗老年股骨粗隆间骨折可以获得相同良好的临床效果，但PFNA手术创伤更小，更适用于高龄并伴有骨质疏松性粗隆间骨折患者。

The treatment results of different operations on the elder patients with intertrochanteric fracture of the femur

Objective To analyze the curative results of 59 cases of intertrochanteric fracture of the femur in old people with dynamic hip screw（DHS） and improved proximal femoral nailanti-rotation（PFNA）. Methods Among 59 eases of intertro- chanteric fracture of the femur, 31 cases were fixed with DHS（ group A）, while 28 cases with PFNA（ group B）. The muhi-index of operation and post-operation between two groups were compared. Results There were remarkable statistic differences between two groups including the operation time, the intraoperative blood loss, postoperative drainage volume, and weight loading-time, the group A was much better than the group B. There was no obvious difference in Hip Functional Evaluation of post-operation. Conclusion Both DHS and PFNA can treat intertrochanteric fracture of the femur in old people effectively, but PFNA is minimal invasive, which is suitable for the elder patients with osteoporosis especially.
