摘    要:目的 探讨第二版前列腺影像报告和数据系统(PI-RADS v2)用于前列腺多参数MRI对前列腺临床显著癌的诊断效能。方法 回顾性收集本院前列腺MR检查数据库资料,筛选临床、影像及病理结果齐全的前列腺疾病初诊患者64例。由1名有经验的影像科医师阅片,根据PI-RADS v2对病灶进行评分。以病理结果为金标准,评价PI-RADS v2对前列腺癌及前列腺临床显著癌的诊断效能。结果 64例患者中,前列腺癌33例(其中临床显著癌29例,非临床显著癌4例),非前列腺癌31例。PI-RADS v2对前列腺癌及前列腺临床显著癌检出的ROC曲线下面积分别为0.95、0.96。PI-RADS v2诊断前列腺临床显著癌的敏感度为96.55%,特异度为88.57%。结论 采用PI-RADS v2对前列腺临床显著癌诊断效能很高,推荐临床常规使用。

Abstract:Objective To evaluate the diagnostic efficacy of Prostate Imaging and Reporting Data System Version 2 (PI-RADS v2) in multiparametric magnetic resonance imaging (mpMRI) for detection of prostate clinically significant cancer. Methods Totally 64 patients with complete clinical information, radiological data and pathological diagnosis were retrospectively recruited. mpMRI results were retrospectively assigned as overall scores using PI-RADS v2 by one experienced radiologist. The diagnostic efficacy of PI-RADS v2 in mpMRI for detection of prostate cancer and clinically significant cancer were measured, with pathological results as gold standard. Results In all of 64 patients, 33 patients were prostate cancer (29 cases were clinically significant cancer, 4 cases were insignificant cancer) and 31 patients were non-prostate cancer. The area under the curve for the detection of prostate cancer and clinically significant cancer with PI-RADS v2 were 0.95 (95%CI), 0.96 (95%CI), respectively. The sensitivity and specificity of PI-RADS v2 in detection of clinically significant cancer were 96.55% (95%CI), 88.57% (95%CI), respectively. Conclusion PI-RADS v2 is demonstrated as a valuable detection tool for prostate clinically significant cancer.
