摘    要:目的 探讨不同病理分子分型乳腺癌三维超声的虚拟器官计算机辅助分析软件(VOCAL)参数差异。方法 回顾性分析经手术病理证实的66例乳腺癌患者术前三维容积超声资料,采用VOCAL进行参数分析,获取平均灰阶值(MG)、平均能量值(MP)、血管指数(R)、血流血管指数(VFI),比较乳腺癌不同病理分子分型(Luminal A型、Luminal B型、HER-2过表达型、基底样型)VOCAL参数的差异。结果 MG值在基底样型最低,HER-2过表达型最高,4型两两比较差异均有统计学意义(P均<0.05);MP在基底样型最低,Luminal B型最高,4型间两两比较差异亦有统计学意义(P均<0.05);R值在Luminal A型最低,Luminal B型最高,其在HER-2过表达型与基底样型、HER-2过表达型与Luminal A型、基底样型与Luminal B型、Luminal B型与Luminal A型间差异均有统计学意义(P均<0.05);VFI在HER-2过表达型与基底样型、HER-2过表达型与Luminal B型、HER-2过表达型与Luminal A型、基底样型与Luminal B型、Luminal B型与Luminal A型差异均有统计学意义(P均<0.05)。结论 不同病理分子分型乳腺癌结节三维超声VOCAL参数具有一定差异。

Abstract:Objective To explore the difference of three dimensional ultrasonic virtual organ computer aid analysis (VOCAL) parameters in patients with breast cancer of different molecular pathology classification. Methods Totally 66 patients with breast cancer confirmed by surgery pathology underwent preoperative three-dimensional ultrasound, and three-dimensional VOCAL parameters were obtained, including mean gray (MG), mean power (MP), vascular index (R), vascular blood flow index (VFI). The differences of MG, MP, R and VFI were compared among different molecular pathological types of breast cancer (Luminal A, Luminal B, HER-2 expression and the basal type). Results The minimum value of MG were in the base type, and highest in HER-2 expression type. Statistically significant differences of MG were found in pairwise comparisons of 4 types (all P<0.05). The minimum value of MP were in the basal type, and highest in Luminal B type. Statistically significant differences of MP was found in pairwise comparisons of 4 types (all P<0.05). The minimum value of R was in Luminal A type, and highest in Luminal B type. Statistically significant differences of R were found between HER-2 expression type and the basal type, HER-2 expression type and Luminal A type, the basal type and Luminal B type, Luminal B type and Luminal A type (all P<0.05). The VFI value in HER-2 expression type and the basal type, HER-2 expression type and Luminal B type, HER-2 expression type and Luminal A type, the basal type and Luminal B type, Luminal B type and Luminal A type had statistically significant differences (all P<0.05). Conclusion Three-dimensional ultrasonic VOCAL parameters have certain differences in different pathology classification breast cancers.
