乳腺脂肪坏死的X线诊断

目的探讨乳腺脂肪坏死的X线表现,提高对脂肪坏死的认识。方法回顾性分析15例经手术、病理证实的乳腺脂肪坏死X线平片表现。结果X线片表现：⑴表现为肿物4例（26.7%）;⑵脂性囊肿2例（13.3%）;⑶表现为小结节2例（13.3%）;⑷脂肪层内索条影交织呈网状2例（13.3%）;⑸脂肪层内星芒影、斑片影、索条影混杂出现3例（20%）;⑹结构不良1例（6.7%）;⑺无任何异常X线表现1例（6.7%）。结论乳腺脂肪坏死根据其病因和时期,其影像表现多种多样,X线表现位于乳腺脂肪层内的网状片影或有毛刺的结节,紧密结合临床病史,首先考虑脂肪坏死的可能,可提高本病诊断准确性,乳腺X线摄影对于脂肪坏死具有诊断价值。

Fat Necrosis of the Breast:Findings on Mammography

Objective To evaluate mammographic findings of fat necrosis of the breast and to improve the imaging diagnosis of the disease. Methods Radiographic findings in 15 cases with pathologically proved breast fat necrosis were retrospectively analyzed.Results The mammographic findings induded （1） Masses （n=4,26.7%）; （2）Single and multiple lipid-cysts （n=2,13.3%）; （3） Nodules （n=2,13.3%）; （4） Faint reticular opacity in the subcutanenous fat layer （n=2,13.3%）; （5）Focal opacities like stars or pieces in the subcutaneous fat layer （n=3,20%）; （6） Structure distortion （n=1,6.7%）;（7）No abnormal mammographic findings （n=1,6.7%）. Conclusion The imaging manifestations of fat necrosis of the breast vary with its etiology and stage. Combined with sonography and clinical information. For a faint reticular opacity or a spiculated nodule in subcutaneous fat layer, fat necrosis should be the first consideration.The mammography is helpful for making the diagnos is of breast fat necrosis.
