新生儿坏死性小肠结肠炎52例疗效观察

目的：探讨５２例新生儿坏死性小肠结肠炎的诊断与治疗。方法：早产儿及足月未成熟儿２８例，足月儿２４例。发病时间为生后２￣７ｄ，５２例均有拒奶，呕吐３８例，腹泻４６例，发热２９例。Ｘ线表现肠腔扩张肠间隙加宽３０例，肠壁积气６例，门脉积气４例，肠梗阻５例，肠穿孔３例。本组５２例中手术１０例，其中肠切除肠吻合４例，肠造瘘６例。其中４２例行保守治疗。本组病例无论手术或非手术治疗均在综合治疗的基础上给予静脉高

CURATIVE EFFECT ON NEONATAL NECROTIZING ENTERO-COLITIS OF 52 CASES

To analyse the diagnoses and treatment of 52 cases of neonatal necrotizing enterocolitis (NEC). Methods: 28 cases were premature infants and small-for-day infants, 24 cases were full-term. The onset time was from the 2nd to 7th day postnatal. All 52 cases suffered from milk refusal, with 38 cases of vomitting, 46 cases of diarrhea, and 29 cases of fever. In their X-ray examinations, 30 cases showed intestinal distention and broadening of interintestinal space, 6 cases displayed pneumatosis intestinales, 5 cases had intestinal obstruction, 8 cases intestinal perforation, and 4 cases portal pneumatosis. Ten of 52 cases were operated, with 4 cases undergoing enterectomy and entero-anastomosis and 6 cases enterostomy. 42 of the, 52 cases were treated with conservative treatment. All cases, either with or without operation, were treated with TPN in addition to compound treatment and TPN solutions were used through peripheral venous transfusion. In this group, TPN was used from 6-30 days under lab surveillance. Results: Am-ong the 52 cases, 47 cases (90. 4%) were cured, including 6 cases undergoing operations, and 5 cases (9. 6%) died. Conclusion: The curative effect of TPN and compound treatment for early diagnosed NEC is quite satisfactory.
