婴幼儿重症感染性细支气管炎85例临床疗效观察

目的 探讨不同的治疗方法佐治婴幼儿重症感染性细支气管炎的临床疗效.方法 对85例住院确诊的重症感染性细支气管炎患儿,采用相同的综合性治疗,包括:吸氧、吸痰、抗感染、化痰平喘等,在上述治疗的基础上随机分为:治疗组甲基强的松龙联合丙种球蛋白( IVIG) 45例,对照组为布地奈德、万托林、爱喘乐联合雾化吸入40例,观察比较两组患儿治疗前后主要症状及体征的持续时间及平均住院时间.结果 治疗组在喘憋缓解、肺部哮鸣音及湿啰音消失、住院时间等方面均明显优于对照组,两组比较差异有显著性(P＜ 0.05).结论 重症感染性细支气管炎婴幼儿应用甲基强的松龙联合IVIG治疗安全、有效,能明显缓解症状及缩短住院时间.

The clinical efficacy for severe infectious bronchiolitis in 85 padiatric patients

Objective To exlore the efficacy of different adjuvant therapies for severe infectious bronchiolitis in padiatric patients. Methods In addition to the comprehensive treament including oxygen inhalation, exsuction of sputum, anti-infection, and dispelling phlegm and relieving dyspnea, 85 padiatric patients diagnosed as severe infectious bronchi- olitis were randomly assigned to receive methylprednisolone and intravenous gammaglobulin （ 45 patients, study group , or inhalation with atmization, budesonide, salbutamol, and ipratropium bromide 40 patients, control group ）. Duration of major symptoms and signs and average length of hospital stay were observed and compared between the two groups. Results The study group was superior to the control group in relief of dysnea, disappearance of wheezing rates and moist rales, and length of hospital stay, with a significant difference （ P〈 0.05 ）. Conclusions Uses of methylprednisolone and intravenous gammaglobulin for padiatric patients with severe infectious bronchiolitis is safe and effective, and can obviously relieve symptoms and shorten length of hospital stay.
