摘    要:目的]探讨阻塞性睡眠呼吸暂停( OSA)患者体重指数和年龄与呼吸暂停低通气指数(AHI)的相关性.方法 对2985例经睡眠多导图(PSG)检查确诊为OSA的患者的体重指数和年龄与呼吸暂停低通气指数进行相关性分析.根据患者年龄分为青年组(18 -44岁)、中年组(45~59岁)和老年组(60~80岁),根据患者体重指数分为正常体重组( 18.5 ~ 23)、超重组(24~27)和肥胖组(>28),比较不同年龄组和不同体重指数组OSA患者PSG的结果.结果 OSA患者AHI与体重指数呈正相关(r=0.445,P<0.001),与患者年龄呈负相关(ι=-0.105,P<0.05).肥胖组AHI( 51.8±27.0)次/h]明显大于超重组( 37.6±23.5)次/h]和正常组( 27.5±21.5次/h)],三组之间的比较差异有统计学意义(P<0.05).老年组AHI( 37.7±23.3)次/h]明显小于中年组( 41.1±25.2)次/h]和青年组( 45.0±28.6)次/h],不同组之间的比较差异有统计学意义(P<0.05).结论OSA患者AHI与BMI呈正相关,肥胖是OSA重要危险因素.

Abstract:Objective To explore the association of body mass index ( BMI ) and age with apnea-hypopnea index ( AHI ) in patients with obstructive sleep apnea ( OSA ). Methods Retrospectively studied 2985 patients with OSA comfirmed by polysomnograhy during the period of 2001 to 2011. The patients were dividing into young adult group ( aged 18 to 44 ), middle-age group ( aged 45 to 59 ), and elderly group ( aged 60 to 80 ) basing on their age, and divided into normal weight group ( 18.5 - 23 ), overweight group ( 24 - 27 ), and obesity group ( 〉 28 ) basing on their BMI. The relationship of AHI with BMI and age was analyzed. Results AHI was positively correlated with BMI ( r= 0.445, P 〈 0.001 ) and age was negatively correlated with AHI ( r = -0.105, P 〈 0.05 ). AHI was significantly greater in the obesity group ( 51.8 ± 27.0 / h ) than in the overweight group ( 37.6 ± 23.5 / h ) and normal group ( 27.5 ± 21.5 / h );, the difference was statistically significant among these three groups ( P〈 0.05 ). AHI was significantly less in the elderly group ( 37.7 ± 23.3 / h ) than in the middle-age group ( 41.1 ± 25.2 / h ) and youth adult group ( 45.0 ± 28.6 / h ); the difference was statistically significant among these groups ( P 〈 0.05 ). Conclusions Apnea-hypopnea index is positively associated with body mass index in patients with obstructive sleep apnea. Obesity is an important risk factor for obstructive sleep apnea.
