直肠癌伴同时性肝脏及腹膜后淋巴结寡转移患者MDT诊治报道

本文介绍1例直肠癌伴同时性肝脏及腹膜后淋巴结寡转移患者的多学科组(multidisciplinary team,MDT)诊治经过.患者就诊时为直肠癌伴有同时性的肝转移及腹膜后淋巴结转移,肝转移病灶属于潜在可切除目标.经过积极的全身治疗后,患者接受切除原发肿瘤以及髂血管旁转移淋巴结的手术.术后反复多次出现疾病的寡进展,通过有效的全身治疗以及多次有效的局部治疗获得长期的生存.该病例诊治经验提示,MDT制度可以给晚期结直肠癌患者提供最优的治疗方案,并帮助患者取得最佳的治疗效果.对于符合寡转移的患者,有效的全身治疗配合有效的局部治疗往往能带来最大的生存获益,而有效的局部治疗包括但不局限于手术治疗.

MDT diagnosis and treatment of rectal cancer with synchronous liver and retroperitoneal lymph node oligometastases

This report described the multidisciplinary team (MDT) diagnosis and treatment process for one case of rectal cancer with synchronous liver and retroperitoneal lymph node oligometastases.The patient was diagnosed as rectal cancer accompanied with synchronous liver and retroperitoneal lymph node oligometastases,and the liver metastatic site was diagnosed as potentially resectable.After active systemic therapy,the patient underwent surgery to remove the primary tumor and lymph node metastases around the iliac vessels.The patient developed multiple oligo-disease progress,but achieved long-term survival through effective systemic therapy and multiple locoregional therapies.Through this case,it is suggested that MDT system can provide late stage colorectal cancer patients with optimal treatment strategy,and help patients to achieve the best therapeutic effect.For patients with oligo-metastases,effective systemic treatment with effective locoregional treatment often leads to the greatest survival benefit,whereas effective locoregional treatment includes,but is not limited to,surgical treatment.
