静脉推注速度对18 F-FES在血管壁残留的影响分析

摘    要:目的:探索静脉推注速度对18 F-雌二醇(18 F-fluoroestradiol,18 F-FES)在血管壁残留的影响及其可能的改进措施。方法分别采用1 min(试验组)和2 min(对照组)静脉推注稀释后的18 F-FES 20 mL,对52例乳腺癌患者行18 F-FES PET/CT显像。观察患者注射前、后生命体征变化,并收集病史资料,从而分析血管壁残留发生的影响因素。结果52例患者中,24例在注射侧血管走形区域内出现显像剂残留,均呈条索状分布。试验组(20例)和对照组(32例)患者不良反应发生率比较差异无统计学意义(χ2=0.332,P>0.05)。试验组18F-FES在血管壁的残留发生率低于对照组(χ2=22.148,P<0.01)。经Logistic回归分析,推注速度可作为独立的影响因子(P<0.01)。血管壁的药物残留在年龄、糖尿病、高脂血症以及近期有无化疗、放疗和内分泌治疗方面比较差异均无统计学意义(均P>0.05)。结论采用20 mL/min静脉推注速度未增加患者不良反应发生率,且可减少18F-FES在血管壁的残留。

Influence of intravenous injection speed on residual 18 F-FES in blood vessel wall
Abstract:Objective To examine the impact of intravenous injection speed on 1 8 F-fluoroestradiol (1 8 F-FES)resid-ual in blood vessel wall during breast PET/CT scan.Methods Twenty milliliter of 1 8 F-FES was administered intravenous-ly with two different speeds [experimental group:1 min (n=20);control group:2 min (n=32)]in 52 breast cancer pa-tients undergoing 1 8 F-FES PET/CT scan.The influencing factors of 1 8 F-FES residual on the blood vessel wall were ana-lyzed.Results The cord-like shape of 1 8 F-FES residual on the blood vessel wall was detected in 24 cases.There was no statistical difference in adverse effect rates between the experimental group and the control group (χ2 =0.332,P>0.05). Nevertheless,the occurrence rate of radiopharmaceutical residual on the blood vessel wall was significantly lower in experi-mental group than that in control group (χ2 =22.1 48,P<0.01 ).Logistic regression analysis showed the speed of intrave-nous injection as an independent factor (P<0.01 ).Besides,age,diabetes mellitus,hyperlipemia and recent history of chemotherapy,radiotherapy or endocrine therapy had no impact on the occurrence rate of 1 8 F-FES residual on the blood ves-sel wall (P>0.05).Conclusion A speed of 20 mL/min of 18F-FES intravenous injection can decrease the occurrence rate of radiopharmaceutical residual on the blood vessel wall without increasing the incidence of any other adverse effect.
