普通外科病人并存糖尿病的围手术期处理

目的　探讨合并糖尿病的普通外科手术病人的围手术期处理方法。方法　对 1997年 3月— 2 0 0 4年 2月我科合并糖尿病的 14 3例普通外科手术病人围手术期控制血糖的治疗方法进行回顾性分析。结果　择期手术 10 6例 ,急诊手术 3 7例。治愈 118例 ,术后发生并发症 2 5例 ,并发症发生率为 17 5 % ,(以切口感染、不愈合多见 ) ,死亡 2例。结论　饮食、口服降糖药和胰岛素治疗相互结合 ,合理控制血糖后 ,外科手术可正常进行 ,应用胰岛素是围手术期糖尿病病人控制血糖的安全有效方法。

The Management of General Surgical Patients with Diabetes during Perioperation

Objective To discuss the managing ways to general surgical patients with diabetes during perioperation.Methods The managing ways to controlling blood sugar of 143 general surgical patients accompanied by diabetes, who were treated by our department from Mar. 1997 to Feb. 2004, were retrospectively analyzed.Results There were 106 patients who were operated at dating time and 37 ones who were treated through emergency operation, among whom 118 were cured and 25 (17.5%) suffered from other complications especially from incision infection. Two patients died. Conclusion Surgical operation can be performed normally on the patients accompanied by diabetes under the condition of controlling blood sugar with combineddiets, sugar-decreasing drugs and insulin during perioperation. To apply insulin is especially stressed.
