门诊妇科病患者支原体的检测及耐药性分析

目的了解妇科和皮肤性病科门诊女性患者支原体感染及耐药性状况。方法统一用解脲脲支原体(Uu)和人支原体(Mh)培养、鉴定、药敏一体化试剂盒,对宫颈拭子标本进行检测。结果2006~2007年检测的775份患者标本,共检出阳性348份,总检出率44.9%,检出支原体共379株,包括Uu 336株(88.7%),Mh 43株(11.3%),其中单一Uu感染占87.6%,单一Mh感染占3.4%,Uu和Mh同时感染占8.9%;多西环素、米诺环素、交沙霉素和克拉霉素对Uu和Mh的敏感率79.8%~92.0%,阿奇霉素敏感率为55.8%~76.5%;喹诺酮类药物的敏感率仅为16.3%~42.9%。结论Uu是主要感染菌种,混合感染Uu和Mh患者的治疗用药应兼顾两种支原体的效果;对于有生殖泌尿道症状或不洁性行为史的女性患者,都有必要检测支原体;四环素类、大环内酯类可作为支原体治疗的首选药物之一,阿奇霉素对Mh治疗效果不佳,喹诺酮类药物因疗效不可靠应慎用。

Detection and Drug Resistance Analysis of Mycoplasma from Gynecologic Outpatients

OBJECTIVE To observe mycoplasma infection and drug resistance status in gynecologic and dermatologic outpatients.METHODS Ureaplasma urealyticum (Uu) and Mycoplasma hominis (Mh) culturing, identification and drug susceptibility integrated kit were used in detection of cervical swab specimen.RESULTS There were 348 cases found positive, with a total detection rate of 44.9% in 775 patient's specimens which were collected from 2006 to 2007.Totally 379 mycoplasma strains were detected, including 336 strains of Uu (88.7%) and 43 strains of Mh (11.1%).single-Uu infection was 87.6%, single-Mh infection was 3.4%, and Uu and Mh mixed infection was 8.9%.The sensitivity rate to doxycycline, minocycline, josamycin and clarithromycin Uu and Mh was 79.8-92.0%.The sensitivity rate to azithromycin was 55.8-76.5%; the sensitivity rate to quinolones was only 16.3-42.9%.CONCLUSIONS Uu is the main pathogen.The treatment for mixed infection of Uu and Mh should take into account the resistance to two kinds of mycoplasma.The female patients who had urogenital symptoms or dirty sexual history have the necessary for mycoplasma detection.Tetracyclines and macrolides are betler choice for treatment of mycoplasma.Azithromycin has unsatisfactory curative effect for Mh infection.Quinolones because of their unreliable efficacy should be used with caution.
