磁共振脂肪抑制技术在鼻咽癌淋巴结转移中的应用

摘    要:目的 总结鼻咽癌颈部淋巴结转移的MRI表现,并探讨MR脂肪抑制技术对发现较小淋巴结转移的作用。方法 收集124例鼻咽癌并颈部淋巴结转移MR脂肪抑制资料,比较冠状面FSE T2WI与STIR淋巴结分布,显示小淋巴结的数量敏感性和特异性。结果 鼻咽癌颈部转移淋巴结的分布以RN区和Ⅱ区居多,并以多区和多个淋巴结广泛转移为主要表现形式;自上向下发展,很少跳跃性转移;其他Ⅲ、Ⅴ、Ⅳ区的转移率逐渐下降。STIR比T2WI多检出小淋巴结89例共167个。STIR图像对转移淋巴结的检出率高于T2WI,两者的检出率差异有显著性(P<0.05)。结论 使用MR脂肪抑制技术有利于鼻咽癌颈部小淋巴结转移灶的显示。

Fat-suppression technique application in lymph node metastasis of nasopharyngeal carcinoma
Abstract:【Abstract】 Objective Summary the MRI performance of lymph node metastasis in nasopharyngeal carcinoma (NPC),and discuss the role of fat-suppression technology in founding small nodes. Methods Collected 124 cases NPC patients with lymph nodes metastasis of fat-suppression MR imaging dates, compared coronal planes of FSE T2WI and STIR sequences of distribution and quantities. Results Cervical lymph node in NPC were mostly located at RN and Ⅱ regions, characterized multiple regions and quantities as the main manifestations of a wide range , developed from the top to the down sides , and rarely skipped ones.167 smaller nodes were found only on STIR sequences in 89 cases. STIR sequence in detecting smaller nodes was higher than T2WI. There was significant difference between the two sequences. Conclusion Fat-suppression technique is help to detect small lymph node metastases in NPC .
