妊娠合并卵巢恶性肿瘤的处理

目的 探讨妊娠合并卵巢恶性肿瘤的治疗方案。方法 对1954年1月－2000年12月，本院收治的17例妊娠合并卵巢恶性肿瘤患者进行回顾性分析。结果 80年代前的11例均行全（或次全）子宫＋双附件切除术或肿瘤活检术。80年代后的6例均为I期病例，其中3例行一侧附件切除术，2例胎儿存活，6例母亲均健在。结论 对妊娠期卵巢恶性肿瘤者，处理原则是尽早手术，并根据具体情况加用化学药物等治疗。

Treatment of ovarian carcinoma in pregnancy

Objective To evaluate the methods of treatment of ovarian carcinoma associated with pregnancy. Methods Retrospective analysis of 17 cases of overian carcinoam associated with pregnancy treated in the Gynecologic and Obstetric Hosptial, Shanghai Medical University between January,1954 and December,2000. Results Before 1980s, total or subtotal hysterectomy and bilateral salpingo oophrectomy were done in 8 cases, adbominal laparotomy or local biopsy were performed in other 3 cases. After 1980s, 6 cases were all stage I cases. Unilateral salpingo oophrectomy was done in 3 cases, two of whom delivered living fetus. The rest 3 cases were treated with panhysterectomy and bilateral salpingo delived living fetus. The rest 3 cases were treated with panhysterectomy and bilateral salpingo oopherectomy and chemotherapy. All of these six patients have been alive till now.Conclusion Definitive or highly suspected overian carcinoma and pregnancy should be operated on as soon as possbile Additional treatment such as chemotherapy etc is also very important when necessary.
