多层螺旋CT曲面重建在面神经管中的临床应用

目的探讨曲面成像技术(CPR)在面神经管疾病中的应用价值。方法回顾性分析慢性中耳炎患者31例、先天性外耳道闭锁患者23例、颞骨外伤患者45例的颞骨薄层CT扫描表现,并将扫描原始数据进行面神经管曲面重建,观察面神经管受累情况。结果 31例慢性中耳炎累及面神经管均有不同程度的骨质改变,CPR图像上主要表现为骨质破坏、中断;先天性外耳道闭锁23例中,面神经管异常达18例,包括面神经管窄短(9例)、行程异位(6例)、骨壁裂缺(3例);颞骨外伤患者45例中,25例可见面神经管损伤,其中,迷路段骨折4例,鼓室部骨折9例,乳突部骨折7例,2例同时损伤乳突段、鼓室段及迷路段,3例同时损伤迷路段及鼓室段。所有征像在CPR图像上均能得到良好展示。结论 CPR技术可以直接用于临床诊断,为治疗面神经管的解剖变异、先天畸形、外伤、中耳炎累及面神经管等,提供有价值的信息。

Clinical Application of MSCT Curved Planar Reconstruction in Facial Nerve Canal

Objective To explore the application value of Curved Planar Reconstruction（CPR） in facial nerve canal disease. Methods 31 cases of chronic suppurative otitis media,23 cases of Congenital concha atresia and 45 cases of temporal bone trauma patients were retrospectively analyzed.The data of thin temporal CT scan shows and the surface reconstruction tube of original data of facial nerve tube were studied.Results Chronic otitis media involving the facial nerve canal had different degrees of bone change,mainly manisfest of CPR image is bone destruction,interruption.In all 23 cases of congenital aural atresia,there are 18 cases of the facial nerve canal anomaly including facial nerve canal narrow-short（9 cases）,stroke ectopic（6 cases）,bone wall crack missing（3 cases）.In all 45 patients of temporal bone trauma,25 cases has neural damage including 4 cases of the labyrinthine segment fracture,9 cases of tympanic fracture and 7 cases of mastoid fracture 2 cases had mastoid segment injury,tympanic segment and the labyrinthine segment injury at the same time.3 cases had both labyrinthine segment and the tympanic segment injury.All the images in CPR had good show.Conclusions CPR techniques can be directly used for clinical diagnosis and provide valuable information for the treatment of facial nerve canal of the anatomic variations,congenital deformity,trauma,otitis media involving the facial nerve canal,etc.
