人血白蛋白联合甲泼尼龙琥珀酸钠对病毒性脑炎患儿脑脊液中SP-D和Gal-9水平的影响

摘    要:目的 探讨人血白蛋白联合甲泼尼龙琥珀酸钠治疗病毒性脑炎的效果。方法 选择濮阳市油田总医院2016年1月至2018年2月收治的病毒性脑炎患儿86例,按随机数字表法分为对照组(n=43)与观察组(n=43)。对照组采用地塞米松治疗,观察组采用人血白蛋白联合注射用甲泼尼龙琥珀酸钠短期冲击治疗,治疗14 d后比较两组患者的临床疗效及治疗前后脑脊液牛乳糖凝集素-9(Gal-9)及肺泡表面活性物质相关蛋白D (SP-D)水平、免疫功能(IgA、IgG、IgM)。结果 观察组总有效率93.02%(40/43),高于对照组的76.74%(33/43),差异有统计学意义(P<0.05);治疗后两组脑脊液Gal-9水平较治疗前降低,脑脊液SP-D水平较治疗前升高,且治疗后观察组脑脊液Gal-9水平低于对照组,脑脊液SP-D水平高于对照组,差异有统计学意义(P<0.05);治疗后两组患者IgA、IgG、IgM水平较治疗前升高,且观察组IgA、IgG、IgM水平高于对照组,差异有统计学意义(P<0.05)。结论 采用人血白蛋白联合注射用甲泼尼龙琥珀酸钠短期冲击治疗病毒性脑炎可有效改善患者脑脊液Gal-9及SP-D水平,调节机体免疫功能,提高治疗效果。

Abstract:Objective To investigate the effect of human serum albumin combined with methylprednisolone sodium succinate in the treatment of viral encephalitis. Methods Eighty-six children with viral encephalitis admitted to Puyang Oilfield General Hospital from January 2016 to February 2018 were enrolled in the study. The patients were divided into control group (n=43) and observation group (n=43) according to the random number table method. The former was treated with dexamethasone, while the latter was with human serum albumin combined with methylprednisolone sodium succinate for injection. After 14 days of treatment, the clinical efficacy, levels of Gal-9 and SP-D in cerebrospinal fluid and immune function (IgA, IgG, IgM) were compared between the two groups. Results The total effective rate of the observation group was 93.02% (40/43), higher than that of the control group 76.74% (33/43) (P<0.05); after treatment, the level of Gal-9 in cerebrospinal fluid of the two groups was lower than that before treatment, the level of SP-D in cerebrospinal fluid of observation group was lower than that of control group after treatment, and the level of SP-D in cerebrospinal fluid of the observation group was higher than that of control group, with significant difference (P<0.05); The levels of IgA, IgG and IgM in observation group were higher than those in control group (P<0.05). Conclusion Early short-term shock therapy with human serum albumin injection combined with methylprednisolone sodium succinate injection for viral encephalitis can effectively improve the level of Gal-9 and SP-D in cerebrospinal fluid, regulate the immune function of the body and improve the therapeutic effect of the disease.
