P53,P16,PRb的表达与宫颈上皮病理改变的关系

目的探讨P16,P53和PRb在宫颈上皮内瘤变和宫颈癌中的表达及意义。方法用免疫组化的方法检测44例宫颈上皮内瘤变,12例宫颈鳞癌和10例宫颈炎标本中P16,P53和PRb的表达情况。结果 P16在宫颈炎、CIN和宫颈癌的阳性表达率为0%,90.9%,100%,其表达有统计学意义（P〈0.01）;P53在宫颈炎、CIN和宫颈癌的阳性表达率为10%,36.1%,41.7%,表达无统计学意义（P=1.00）;PRb的表达率为0%,95.5%,100%。（P〈0.01）具有统计学意义。从宫颈炎-CIN-宫颈癌的演变过程中,P16和PRb的表达成递增趋势,具有高度相关性（r=0.807,0.668）,而P53的表达与疾病演变过程呈低度相关性（r=0.331）。结论 P16和PRb的表达强度在宫颈CIN到宫颈癌的演变过程中逐步升高,P53表达则主要出现在病变的早期阶段。P16,PRb和P53结合应用有助于宫颈CIN分级和宫颈癌的诊断及鉴别诊断。

Expression and significiation of P16,P53 and PRb in cervical intraepithilial neoplasia and cervical carcinoma

Objective To investigate the expression and signification of P16,P53 and PRb in cervical intraepithilial neoplasia and cervical carcinoma.Metheds Expression of P16,P53 and PRb were detected by immunohistochemistry method in 44 cases of cervical intraepithilial neoplasia（CIN）,12 cases of cervical squarmas carcinoma and 12 cervicitis.specemens.Results In cervicitis,CIN and cervical carcinom,the postive rates of P16 were 0%,90.9% and 100%,respectively with significant difference（P0.01）;in cervicitis,CIN and cervical carcinoma,the postive rates of P53 were 10%,36.1% and 41.7% respectively without significant difference（P=1.00）;the positive rate of PRb were 0%,95.5% and 100%,respectively with significant difference（P0.01）.From the progress of cervicitis-CIN-carcinoma,the expression intensity of P16 and PRb were increased gradually and there were significant correlation betweet them（r=0.807,0.668）.While there was no significant correlation in expression of P53.（r=0.331）.Conclusion The expression levels of P16 and PRb were increased with cervical epithlelial malignant transformation progress and the expression of P53 was the early event of develepment of cervical carcinoma.Using P16,PRb and P53 may be helpful in the differential diagnosis of cervical squaremas carcinoma and CIN grades.
