CT定向钻孔引流治疗高血压脑出血

目的:评价CT定向钻孔引流治疗高血压脑出血的疗效及适应证。方法:对比分析自1998年-2006年间采用CT定向钻孔引流术和小骨窗开颅术治疗高血压脑出血患者术中及术后病死率、术后并发症、手术持续时间、血肿清除程度、拔管时间及近期生存质量的差别。结果:CT定向钻孔引流术与小骨窗开颅相比,前者在手术持续时间、术后并发症及拔除引流管时间等方面明显优于后者,两者在术后死亡率及近期生存质量两方面无显著差别,但在血肿清除率方面后者明显优于前者。结论:对于出血量较少的基底节区高血压脑出血(<60ml),CT定向钻孔引流术可以达到与小骨窗开颅血肿清除术相似的治疗效果,并且有手术时间短、术后并发症少及拔管时间短等优点,但是手术方式的选择则应以患者的具体情况为依据。

The Treatment of Hypertensive Intracerebral Hemorrhage with Minimally Invasive Trepanation and Drainage

Objective:To evaluate effectiveness and reliability of minimally invasive trepanation and drainage.Methods:The chinicel data of 89 patients with minimally invasive trepanation and drainage and 42 patients with small bone window craniotomy in our hospital were reviewed retropectively.Results:No statistical difference in the rate of death rate and quality of life in near futrue were found between the two groups,minimally invasive trepanation and drainage group had an obvious advantage in operation time,complication and extubation time,but small bone window craniotomy group had the predominance in the rate of clerance degree of hematoma.Conclusion:The operation of minimally invasive trepanation and drainage is an effective and safty modus operandi in small quantity of hematoma.
