妊娠合并肝硬化的妊娠安全性及其结局探讨

目的:探讨妊娠合并肝硬化的妊娠结局与孕产期处理。方法:回顾分析妊娠合并肝硬化33例临床资料。结果:33例孕妇中代偿期肝硬化孕妇27例,失代偿期肝硬化孕妇6例,失代偿期肝硬化孕妇死亡2例(33.3%),其中1例为中孕流产,另1例为足月顺产。33例妊娠合并肝硬化孕妇孕早中期流产9例(27.2%),早产12例(50%);无新生儿死亡;阴道分娩4例(16.7%),剖宫产20例(83.3%)结论:妊娠合并肝硬化严重威胁母儿的安全,失代偿期肝硬化患者不宜妊娠。适时终止妊娠是治疗妊娠合并肝硬化的重要手段,终止妊娠的方式首选剖宫产。

Perinatal Management and Pregnancy Outcome in Pregnant Women with Hepatic Cirrhosis Disease

Objective:To investigate the relationship between the outcome and method of pregnancy termination in pregnancy women with hepatic cirrhosis disease.Methods:Retrospective analyses were done on 33 patients with hepatic cirrhosis disease in latest 10 years.Results:Among these 33 patients,27 pregnancy women were with compensated cirrhosis,6 women were with decompensated cirrhosis.A prengnency woman with decompensated cirrhosis had higher mortality than compensated cirrhosis 33.3%,one aborted in the second trimester prestation,one had vaginal delivery.The rate of abortion was 27.2%,preterm labor 50%.No newborn died.The vaginal delivery was lower 16.7%,the cesarean sections was higher 83.3%.Conclusion:Hepatic cirrhosis disease is dangerous to both mother and perinatal fetus.Women with decompensated cirrhosis are not suitable to pregnency.Timely terminating pregnancy is one of the most important treatment methods for pregnancy with hepatic cirrhosis disease.Cesarean section is the first choice.
