摘    要:目的 探讨乳腺癌组织中表皮生长因子受体(epidermal growth factor receptor,EGFR)的分布特点及其与乳腺癌的发病年龄、组织学分级、腋窝淋巴结有无转移、雌孕激素受体及人表皮生长因子受体2(human epidermalgrowth factor receptor-2,HER2)的关系,为乳腺癌的临床诊断、治疗及预后提供重要依据.方法 利用免疫组化法检测144例乳腺癌组织中EGFR的表达情况.结果 ①144例乳腺癌组织中EGFR(+)表达百分率为38.2%.②EGFR的表达与患者年龄大小、淋巴结转移与否和组织学分级Ⅰ与Ⅱ、Ⅱ与Ⅲ之间比较差异无统计学意义(P>0.05);但在组织学分级Ⅰ与Ⅲ,淋巴结转移<4枚与淋巴结≥4枚的患者间比较,差异有统计学意义(P<0.05).③EGFR的表达与ER、PR之间比较,差异无统计学意义(P>0.05).HER2(-)与HER2(+)、HER2(++)与HER2(+++),差异无统计学意义(P>0.05),但HER2(-)与HER2(++)、HER2(-)与HER2(+++)、HER2(+)与HER2(+++)之间比较,差异有统计学意义(P<0.05).结论 ①EGFR的表达与ER、PR表达状况无明显相关.但EGFR在HER2强阳性乳腺癌患者中的表达明显高于HER2弱阳性患者,可能与乳腺癌侵袭、转移和预后存在关联性.②EGFR的表达在淋巴结转移<4枚与淋巴结≥4的患者间比较,差异有统计学意义(P<0.05),随着淋巴结转移数量的增加,其阳性表达率明显增高.

Abstract:Objective To investigate EGFR distribution characteristic in the breast Cancer and its relationship with the breast cancer age, histological grad, with or without lymph node metastasis, estrogen and progesterone receptor so as to provide an important basis on diagnosis, clinical treatment and prognosis of the breast cancer.Methods Material taking and eosin staining, detect the expression of EGFR in 144 cases of breast cancer with the method of immunohistochemistry. Analyze the expression of EGFR in breast cancer tissue.Results (1)The positive rate of EGFR is 38.2% in 144 breast cancer tissues,infiltrating ductal carcinoma 102 cases, invasive Iobular carcinoma 26 cases. (2)The relationship between the expression of EGFR menopause, histological grade,with or without lymph node metastasis,pathological grade Ⅰ and Ⅱ, Ⅱ and Ⅲ are not statistically significant (P〉O. 05) ,but the comparison of histological grade Ⅰ and Ⅲ, lymph node metastasis is less than 4 and greater than or equal to 4 significant differences(P〈 0. 05).(3)There is no difference(P〉0. 05) in the comparison between EGFR expression on ER, PR results but in the ER (-), PR (-) of the patients, positive expression rate of EGFR is low; there was no statistically significant difference (P〉0. 05) in HER2 (-) and HER2 (+), HER2 (++) and HER2 (+++) expression,but in HER2 (-) and HER2 (++),HER2 (-) and HER2 (+ ++) ,HER2 (+) and HER2 (+++),statistically significant difference (P〈0. 05).Conclusion (1)The expressions of EGFR are not related to ER and PR status.The expressions of EGFR in HER2 high positive breast cancer are more than in the negative breast cancer. That suggests the expression of EGFR is correlated to the invasion, metastasis and prognosis of breast cancer.(2) lymph node metastasis is less than 4 and greater than or equal to 4 significant differences(P〈 0. 05).
