摘    要:①目的比较胃食管经食管床主动脉弓后吻合术式与传统的经胸腔主动脉弓前至弓上吻合术式对中段食管癌病人肺功能和胃排空的影响。②方法将40例中段食管癌病人随机分成2组,每组20例,分别采用传统手术方法和经食管床弓后吻合法治疗。检测病人术前、术后14d的肺功能,并应用放射性核素测定术后病人的胃半排时间。③结果肺活量、最大通气量、1秒用力呼气容积、用力呼气中期流速以及胃半排时间两组比较有显著性差异(t=3.23~9.47,P〈0.05)。④结论弓后法可缩短病人胃排空时间,对肺功能影响较小。

Abstract:Objective To compare the effects of two operation types of esophagogastrostomy(the type behind the esophagus and aortic arches and the traditional type before and upon the aortic arches) on pulmonary function and gastric excretory time in patients with mid- esophageal cancer. Methods Forty patients were randomly divided into two groups, 20 patients for each, and were treated respectively with one of the two operation types mentioned above. Before and after the operation, all patient's pulmonary function were detected and in the same time, the half gastric excretory time was examined by the assay of radionuclide clearance technique. Results There were significant differences in patient's vital capacity, maximal ventilative volume, first-second forced expiratory volume, forced mid--expiratory flow and half gastric excretory time between the two groups(t=3.23-9.47, P<0.05). Conclusion The type of esophagogastrostomy behind the esophagus and aortic arches can reduce the time of gastric excretion and has less influence on pulmonary function.
