2型糖尿病胰岛素抵抗与脂肪肝的关系

目的 探讨2型糖尿病患者胰岛素抵抗与脂肪肝的关系.方法 对154例2型糖尿病患者进行体重指数(BMI)、空腹血糖、空腹胰岛素水平、糖化血红蛋白、血脂、肝功能和B超检查.根据B超检查结果分为伴脂肪肝组与不伴脂肪肝组.结果 伴脂肪肝组的体重指数、甘油三酯、低密度脂蛋白、空腹胰岛素水平均明显高于不伴脂肪肝组(P＜0.05);而脂蛋白(a)(Lp-a)则明显降低(P＜0.05).结论 2型糖尿病伴脂肪肝患者存在明显的胰岛素抵抗及脂质代谢紊乱,积极控制血糖、血脂,减少胰岛素抵抗,有利于防止脂肪性肝炎和肝硬化发生.

Relationship between insulin resistance and fatty liver in type 2 diabetes

Objective To investingate the relationship between insulin resistance and fatty liver in type 2 diabetes.Methods BMI,fasting blood glucose,insulin,glycohemoglubin,lipids,liver function tests and examination of type B ultrasonography were detectcd in all 154 patients with type 2 diabetes.They were divided into two groups with and without fatty liver according to the examination of type B ultrasonography.Results BMI,the levels of triglyceride,low density lipoprotein and fastiong blood insulin of the patients with fatty liver were significantly higher than those of the patients without fatty liver(P<0.05),but the level of a lipoprotein was lower than that of the patients without fatty liver(P<0.05).Conclusion Obvious insulin resistance and disorder of lipid metabolism exist in the type 2 diabetes with fatty liver.Controlling the blood glucose and lipid for decreasing the insulin resistance might be useful to prvent the steatohepatites and hepatic cirrhosis.
