2型糖尿病患者血清总胆红素与血脂、血小板参数和红细胞分布宽度的相关性研究?

目的：探讨2型糖尿病(T2DM)患者血清总胆红素(TBIL)水平与血脂、血小板参数和红细胞分布宽度(RDW)的关系。方法选择2012年8月至2014年1月在泸州医学院附属医院内分泌科就诊的497例 T2DM患者，同时选择75例正常人作为对照组(NC 组)，采用酶法测定血清 TBIL 水平，并分析血清 TBIL 与血脂、血小板参数和 RDW 等指标的关系。结果 T2DM 患者血清 TBIL 水平明显低于 NC 组(P <0.01)；T2DM 患者血清TBIL 水平与直接胆红素(DBIL)、间接胆红素(IBIL)和血小板计数(PLT)呈明显正相关，与 RDW、血小板平均体积(MPV)和血小板比容(PCT)呈明显负相关(P <0.05或 P <0.01)；RDW 和 MPV 是影响 T2DM 患者血清 TBIL水平的独立相关因素。结论T2DM 患者血清 TBIL 水平与血脂无关，与血小板参数和 RDW 密切相关，可能在T2DM 的发生发展中具有重要作用。

Relationships between Serum TBIL,Blood Lipid,Platelet Parameters and Red Cell Distribution Width in Patients with Type 2 Diabetes Mellitus

Objective To investigate the relationships between level of serum total bilirubin (TBIL),blood lipid,platelet parameters and red cell distribution width (RDW)in patients with type 2 diabetes mellitus (T2DM). Methods 497 patients with T2DM and 75 normal subjects (NC group)were selected and their respective fasting serum TBIL levels were measured by enzymatic method and the relationships between the change of TBIL,blood lipid,platelet parameters,RDW and other parameters were analyzed.Results Serum TBIL levels in T2DM patients were significantly lower than those of NC group (P <0.01 ).Serum TBIL levels correlated positively with direct bilirubin (DBIL),indirect bilirubin (IBIL)and platelet count (PLT),and negatively correlated with red cell distribution width (RDW),mean platelet volume (MPV)and platelet pressure accumulates (PCT)in T2DM patients (P <0.05 or P <0.01).Multiple regression analysis showed that RDW and MPV were independent related factors influencing serum TBIL levels.Conclusion Serum TBIL levels in patients with T2DM are not related with blood lipid while well related with platelet parameters and RDW,which can play an important role in the occurrence and development of T2DM.
