卵巢恶性肿瘤的CT诊断价值

目的探讨CT在卵巢恶性肿瘤中的诊断价值。方法回顾性分析经手术和病理证实的36例卵巢恶性肿瘤的CT表现和特点,包括病灶的CT分型及伴随征象(腹腔积液、腹膜转移、远处转移等)。结果36例卵巢恶性肿瘤,双侧病灶28例(占77.8%),单侧病灶7例(占19.4%),1例CT未发现病灶。共65个病灶,CT发现病灶60个,其中Ⅰ型4个,Ⅱa型12个,Ⅱb型20个,Ⅱc型22个,Ⅲ型2个。术前CT正确诊断57个(占87.7%),误诊3个(占4.6%),漏诊5个(占7.7%)。伴随征象主要为腹腔积液(28/31);腹膜种植,表现为大网膜(18/23)、子宫直肠窝(9/17)、右膈下(3/9)、肠系膜(7/13)、肠道侵犯(6/16)等。远处转移主要表现为肝转移(2/2)或淋巴结肿大(2/3)。结论CT扫描,尤其是增强扫描对卵巢恶性肿瘤的定性及定位诊断具有一定的意义。

The CT diagnostic value of malignant ovarian tumors HUA NG

Objective To estimate the value of CT in the diagnosis of malignant ovarian tumors. Methods CT features of 36 cases of histologically proven malignant ovarian tumors were retrospectively reviewed, included CT types of the lesion and following signs （such as ascites, peritoneal seeding, metastatic lesions）. Results Total 66 lesions of 36 patients were analyzed in our group, 60 lesion were detected on CT scan. The tumors involved bilateral sides in 28 cases （77.8%） and unilateral side in 7 cases （19.4%）. One patient did not show any lesions on CT scan. Of the 60 lesions, type Ⅰ , Ⅱ a , Ⅱ b , Ⅱ c and Ⅲ was 4, 12, 20, 22 and 2 cases respectively. The CT diagnosis were made correctly in 57 cases （87.7%）; misdiagnosis in 3 cases （4.6%）, missed diagnosis in 5 cases （7.7%）. Following signs included ascites （ 28/31 ）, involvement of greater omentum （ 18/23 ）, utero-rectal pouch （ 9/17 ）, fight infra-diaphragm （3/9）, mesentery （7/13）, intestinal duct （6/16） and so on. Metastatic lesions included liver involvement, enlargement of lymph nodes. Conclusion CT scans, especially contrast enhancement is of important value in the qualitation and position diagnosis of malignant ovarian tumors.
