经皮外周静脉插管置管术对老年晚期肿瘤患者生活质量的影响

目的探讨经皮外周静脉插管（PICC）置管术对老年晚期肿瘤患者生活质量的影响。方法选取2010年2月至2012年2月收治的86例老年晚期肿瘤PICC置管术化疗患者，按照置管静脉不同分为观察组与对照组，每组43例。观察组患者置管静脉为上臂静脉，对照组患者给予常规肘部静脉，比较两组患者的相关指标。结果两组患者置管平均留置时间差异无统计学意义（t=1．343，P=0．091）；观察组患者并发症及意外事件发生率分别为4．7％（2／43）和2．3％（1／43），均显著低于对照组患者的23．3％（10／43）和14．O％（6／43）差异无统计学意义（χ^=6．198、3．888，P=0．013、0．048）；观察组患者生活质量良好率为95．3％（41／43），高于对照组患者的60．5％（χ^2=15．200，P=0．000）；年龄、治疗效果、疾病分期是影响PICC置管患者生活质量的主要因素。结论PICC置管术提高了晚期肿瘤患者的生活质量，降低了并发症发生率，年龄〈70岁、治疗效果良好、疾病程度较轻患者的生活质量较好。

Analysis of peripherally inserted central catheter on the quality of life of elderly patients with ad- vanced cancer

Objective To explore the PICC catheter impact on the quality of life of elderly patients with advanced cancer and factors affect the quality of life of patients. Methods 86 elderly patients with ad- vanced cancer patients were divided into observation group and control group. Observation group adopted the upper arm vein catheter vein, the control group conventional elbow vein. Related indicators of the two groups were compared. Results The average retention time of two groups was not statistically significant （t = 1. 343 ,P = 0. 091 ） ; The observation group of complications and accidents [ 4. 7% （ 2/43 ） and 2. 3% （ 1/43 ） respectively] were lower than those of the control group [ 23. 3% （ 10/43 ） and 14.0% （ 6/43 ） respectively, （χ^2 =6. 198,3. 888 ,P =0. 013,0. 048）]. The wellbeing rate of quality of life of the observation group was 95.3% （41/43） ,which was higher than 60. 5% of the control group （χ^2 = 15. 200,P = 0. 000） ; age, treatment and disease stage were the main factors affecting the quality of life of patients with PICC catheter. Conclusions PICC catheter improved the quality of life of patients with advanced cancer, reduced the incidence of complications. Patients under 70 years old at tower disease stage with better treatment effect were more likely gained better quality of life..
