| 摘 要： | 目的比较75岁以上慢性肾衰竭患者行血液透析（HD）及血液透析滤过（HDF）的可行性和有效性。方法将40例75岁以上慢性肾衰竭患者随机分为2组,A组（HD组）应用每周3次标准HD治疗;B组（HDF组）行每周1次常规标准HD治疗和每周2次HDF治疗。比较2组患者治疗前及治疗12周后甲状旁腺素（iPTH）、血压控制、贫血情况,观察治疗期间心血管并发症发生情况。结果 B组患者iPTH、血磷清除、平均动脉压下降更加显著,贫血纠正优于A组,低血压、心绞痛、心律失常、心衰的发生率明显降低。结论 HD及HDF是治疗高龄（≥75岁）慢性肾衰竭患者的有效手段。血液透析滤过对老年尿毒症患者有较好的心血管稳定性,值得临床推广。
