卡前列甲酯栓联合催产素防治高危产妇剖宫产术后出血的疗效观察

目的观察卡前列甲酯栓联合催产素防治高危产妇剖宫产术后出血的临床效果。方法选择我院妇产科2008年1月-2010年12月间200例行剖宫产手术的高危产妇,随机分为研究组和对照组,每组100例,研究组产妇在术中打开腹膜时给予卡前列甲酯栓1 mg舌下含化,胎儿娩出后给予宫体肌肉注射催产素20 U;对照组产妇仅在胎儿娩出后给予宫体肌肉注射催产素20 U。观察术中、术后2 h内产妇出血量及出血总量。结果研究组100例高危产妇术中出血量为（302±178）ml,产后2 h内出血量为（108±78）ml,出血总量为（408±207）ml;对照组100例高危产妇术中出血量为（428±206）ml,产后2 h内出血量为（141±92）ml,出血总量为（558±285）ml。两组间差异有统计学意义（P〈0.05）。研究组出血总量≥1 000 ml的产妇有3例（3%）,对照组出血总量≥1 000 ml的产妇有6例（6%）。结论卡前列甲酯栓联合催产素能有效的防治高危产妇剖宫产术后出血,其作用优于单独使用催产素,值得临床推广。
