脑卒中急性期血糖升高病人恢复期口服糖耐量试验时血糖、胰岛素和胰岛素敏感性指数的分析

目的　探讨脑卒中急性期血糖升高患者是否存在胰岛素抵抗现象。方法　64例脑卒中患者分为急性期糖化血红蛋白正常空腹血糖升高组(血糖升高组)32例,糖化血红蛋白正常空腹血糖正常组(对照组)32例;治疗1月后,测定其口服糖耐量试验时空腹和服糖后30,60,120和180min时血糖、血胰岛素水平,计算其胰岛素敏感性指数(ISI)。结果　血糖升高组和对照组空腹血糖,服糖后120,180min时的血糖无变化(P>0.05);服糖后30,60min时的血糖升高组较对照组高(P<0.05);血糖升高组空腹胰岛素水平比对照组高(P<0.05),胰岛素敏感性指数比对照组低(P<0.05);服糖后血胰岛素水平比对照组明显升高(P<0.01),胰岛素敏感性指数比对照组低(P<0.05)。结论　脑卒中急性期血糖升高患者存在胰岛素抵抗,胰岛素抵抗可能是脑卒中患者急性期血糖升高的一个原因。

Measured the Blood Glucoses,Insulin and ISI by Using Oral Glucose Tolerance Test in Hyperglycemic Patients of Stroke in the Acute Stage

Objective To determine whether There is insulin resistance in hyperglyemic patients with stroke in the acute stage. Methods 64 cases stroke were selected, 32 cases normal glucohemoglobin and high fasting blood glucose ( FBG ) as hyperglycemia group, other 32 cases normal glucohemoglobin and FBG as control group. The blood glucose, insulin levels and insulin sensitivity index (ISI) were measured by using oral glucose tolerance test ( OGTT) after one month with treated. Results The hyperglycemia group had a higher fasting insulin level and had a lower ISI than the control group (P<0.05 ). After oral glucose, the hyperglycemia group had siginficantly higher plasma insulin levels and had a lower ISI than the control group (P<0.01, P<0.05 ). Conclusion In hyperglycemic patients with stroke in the acute stage there are hyperinsulinemia and insulin resistanee.
