经皮肾穿刺碎石术治疗输尿管上段结石52例临床分析

目的 探讨经皮肾穿刺碎石术（percutaneous nephrolithotomy,PCNL）治疗输尿管上段结石的经验和疗效.方法 回顾分析52例输尿管上段结石采用PCNL治疗的临床资料.结果 50例一次性取尽结石,成功率96％,2例结石碎片下移,术毕采用输尿管镜取出碎片.手术时间平均36分钟,术后2例发热高于38.5℃,应用抗生素后愈,无需要输血病例.住院时间平均6天,随访2月～半年,无肾功能不全病例出现,52例肾积水均有不同程度减轻.结论 PCNL治疗输尿管上段结石具有创伤轻微,疗效可靠等优点,值得推广应用.

Clinical analysis of treat upper ureteral calculus with percutaneous nephrolithotomy of 52 patients

Objective To evaluate the experience and efficiency of percntaneous nephrolithotomy in treating upper ureteral calculus. Methods Retrospective analyzes the clinical data of 52 cases with upper ureteral calculus and received percutaneous nephrolithotomy. Results The stones of 50 patients were cleared off at one section, the effective rate was 96%. 2 patients with residual stones after operation were cured by URSL. Average operative time was 36 minutes. Two patients got fever （ 〉 38.5°C ） after operation were cured by antibiotics and no patient need blood transfusion. The mean hospitalization was 6 days, all the patients were followed up for 2 -6 months, none renal faihlre occurred and the hydronephrosis of all patients alleviated after operation. Conclusions Percutaneous nephrolithotomy is worth generalizing in treating upper ureteral calculus for it had advantages as less invasion, stable efficacy.
