摘    要:目的 应用三维多普勒频谱定量分析技术检测2型糖尿病(T2DM)患者拇指动脉血流,评估T2DM患者外周微小动脉血流的特点。方法 本研究根据T2DM的病程,将所有患者分为T2DM组1(病程≤5年;n=47)与组2(病程>5年;n=45)两组,46例正常志愿者为对照组。应用三维多普勒频谱定量分析技术的流速-血管深度频谱图模式,测定拇指动脉内径及拇指动脉频谱图收缩期瞬时最大血流面积(Aflow)。并且测定收缩期最大血流速度(PSV)、舒张末期血流速度(EDV)、阻力指数(RI)。结果 所有T2DM患者拇指动脉血管内径均较对照组缩小[正常人拇指动脉血管内径平均为(2.11± 0.19)mm,T2DM组1平均为(1.71±0.21)mm,T2DM组2平均为(1.59± 0.25)mm,P<0.001];T2DM两组患者的拇指动脉Aflow较对照组显著减低[T2DM组1平均为(61.35±10.66)mm2/s,T2DM组2平均为(46.50±6.59)mm2/s,对照组平均为(75.32±11.97)mm2/s,P<0.001]。而且T2DM组2的拇指动脉Aflow较T2DM组1更为减低(P<0.001)。结论 三维多普勒超声技术所测拇指动脉内径与相关血流参数,可应用于临床观测T2DM患者外周微小血管的损伤度,为观测糖尿病患者外周血管病变提供一种可靠的无创伤性分析手段。

Abstract:Objective To evaluate changes in peripheral arterioles in patients with type 2 diabetes (T2DM) in vivo with quantitative three-dimensional Doppler spectrum on measurement of artery flow at the tip of the right finger. Methods All of the 92 patients with T2DM were divided into two subgroups as T2DM group 1(n=47) and T2DM group 2(n=45) according to the disease progress. Forty-six healthy subjects were selected as control group. Diameter of finger arterioles at right thumb, maximal area of instantaneous flow in finger arterioles from the flow spectrum (Aflow), peak systolic velocity (PSV), end diastolic velocity (EDV) and resistance index (RI) were calculated from the quantitative three-dimensional Doppler spectrum. Results Compared with control group, diameter of the finger arterioles of T2DM patients were decreased significantly ([1.71±0.21]mm in T2DM group1,[1.59±0.25]mm in T2DM group 2,[2.11± 0.19]mm in control group, P<0.001), and Aflow of T2DM patients were diminished significantly ([62.18±10.31]mm2/s in T2DM group 1,[45.36±7.02]mm2/s in T2DM group 2,[75.32±11.97]mm2/s in control group, P<0.001). Aflow was lower in T2DM group 2 than that in T2DM group 1 with significant statistical differences (P<0.001). Conclusion Diameter of the finger arterioles and its flow parameters by quantitative three-dimensional Doppler spectrum might be a reliable noninvasive method to observe damage of peripheral arterioles in vivo, which could provide quantitative analysis for peripheral arterioles in patients with T2DM.
