皮层脑电图监测在颅脑病变伴发癫(癎)手术中的应用

目的 探讨皮层脑电图监测在颅脑病变伴发癫(癎)手术中的应用价值.方法 回顾分析58 例皮层脑电监测下手术切除病变及致(癎)灶患者临床资料.结果 所有患者在切除病变后复查皮层脑电图仍有(癎)样放电,其中37 例行扩大皮层切除,18 例加行皮层热灼术,3 例加行MST.术后1 /2 ～6 年随访发现癫(癎)控制达到EngelⅠ级32 例,EngelⅡ12 例,Engel Ⅲ级3 例,Engel Ⅳ 11 例,术后癫(癎)控制有效率为84.48%.结论 对于颅脑病变伴发癫(癎)的患者,应在皮层脑电图监测下进行手术,且在切除病变的同时一并将致(癎)灶切除.

Application of electrocorticography monitoring in the surgery of intracranial lesions accompanied with epilepsy

Objective To explore the application value of eleetrocorticography monitoring in the surgery of intracranial lesions accompanied with epilepsy. Method The clinical data of 58 patients with intracranial lesions ccompanied with epilepsy performed the resection under electrocorticography monitoring were analysized respectively . Result After the resection of the lesion there were still discharges in all the cases, among whom enlarged cortical resection were performed in 37, cortical coagulation in 18 and multiple subpial transaction（MST） in 3. The postoperative 1/2 to 6 years follow-up showed seizure control of Engel Ⅰ in 32, Engel Ⅱ in 12 cases, Engel Ⅲ in 3 cases, and Engel Ⅳ in 11 cases. The overall effective rate was 84.48%. Conclusions For the patients of intracranial lesions accompanied with epilepsy, the operation should be performed under electrocorticography monitoring and the epileptic lesions should be resected at the same time.
