心脏外科手术中过敏患者的血浆胶体渗透压变化

目的研究体外循环中过敏患者的血浆胶体渗透压变化。方法测定7例过敏患者的血浆胶体渗透压、血常规及血压变化,分析其变化规律。结果过敏后患者血压、血浆胶体渗透压显著降低,血浆胶体渗透压与血压变化一致,过敏纠正后缓解。结论过敏后毛细血管壁通透性增高、血管扩张,血浆蛋白和胶体物质透出血管壁,导致血浆胶体渗透压降低,治疗上应该缩血管药物、肾上腺皮质激素和扩容三者并重。

Changes of Plasma Colloid Osmotic Pressure in Allergic Patients during Heart Surgery

OBJECTIVE To review the changes of plasma colloid osmotic pressure in allergic patients during cardiopulmonary bypass. METHODS Plasma colloid osmotic pressures, routine blood tests and blood pressure were measured in seven allgeric patients. RESULTS Due to the allergy, plasma colloid osmotic pressure, paralleled with blood pressure, decreased significantly in all of patients and backed to normal level after allergy improvement. CONCLUSION Increasing capillary permeability, vasodilation and leakage of protein and other colloids decrease plasma colloid pressure while allergy occurs. Treatment combination with vasoconstrictor, steroid and volume support should be necessary.
