延续护理在肝移植患者中的应用

目的总结在肝移植患者中开展延续护理服务的经验。方法以我院随访系统中纳入的435例肝移植患者为延续护理服务的对象,介绍为这些肝移植患者提供延续护理的方法及内容。结果 100%的患者出院后能够正确服用药物,92%的患者具备了较好的自我监护能力,按时复诊率达到99.5%,满意度达到99.8%。患者总存活率为74.4%,术后排斥反应率为8.0%。结论在肝移植患者中开展延续护理服务满足了患者出院后的健康需求,提高了患者的生存质量,并且增加了患者对医院医疗和护理服务的满意度。

Extended care for liver transplant discharged patients

Objective To summarize the experience in offering extended care for liver transplant discharged patients . Methods A total of 435 liver transplant discharged patients enrolled in the follow-up system of our hospital were given extended care services. How the extended care services were provided and what the extended care services consisted of were introduced. Results All the patients could take medicine correctly at discharge, and 92% of the patients developed a good self-care ability. Further consultation rate was 99.5% and patient satisfaction rate was 99.8%. The total survival rate was 74.4%, and the liver trans-plant rejection rate was 8.0%. Conclusions Offering extended care for liver transplant discharged patients meets their health-care needs, improves their quality of life, and increases their satisfaction with hospital＆#39;s medical services and nursing care.
