尿胰蛋白酶原激活肽对急性胰腺炎的诊断价值

目的评价尿胰蛋白酶原激活肽(TAP)对急性胰腺炎(AP)特别是重症急性胰腺炎(SAP)的早期诊断价值。方法对30例急性胰腺炎患者(其中10例为重症AP，20例为轻型AP)及30例其它急腹症病人同时进行尿胰蛋白酶原激活肽、血清脂肪酶及淀粉酶肌酐清除比率的检测。结果急性胰腺炎组与非急性胰腺炎组各项检测结果均存在显著性差异，但各指标检测特异性、敏感性存在一定差异。在尿TAP含量的检测中，重症AP组与轻型AP及非急性胰腺炎组间均有显著性差异，TAP预测重症AP的敏感性、特异性、阳性预测值、阴性预测值、诊断准确度分别为100％，75．0％，66．7％，100％，83．3％。结论与其它检测指标相比，尿TAP浓度的检测能更精确及早期预测SAP，能够反映胰蛋白酶原的激活程度。

Diagostic Value of Urinary Trypsinogen Activation Peptide for the Acute Pancreatitis

Objective To investigate whether TAP can be used for the earl y predication of acute pancreatitis, especially of severe acute pancreatitis. Me thods In this study, we examined the concentration of urinary trypsinogen activ ation peptide, blood lipase and the ratio of CAmy/Ccr in 30 acute pancreaticis p atients (10 with severe disease, 10 with mild disease) and 30 control cases. Res ult There was a marked difference in the concentration of urinary trypsinogen a ctivation peptide among severe AP, mild AP and controls group. The sensitivity, specificity, positive predictive, negative predictive values and accuracy of usi ng urinary trypsinogen peptide to identify severe acute pancreaticis on admissio n were 100%,75.0%, 66.7%, 100%, 83.3% respectively. Conclusion Compared with oth er test, urinary TAP can be used to predict SAP accurately, and the concentratio n of TAP has good correlation with severity of the disease.
