2型糖尿病与脂肪肝、胆囊炎、胆结石和脂代谢的相关因素分析

摘    要:目的:探讨糖尿病与脂肪肝、胆囊炎、胆结石和脂代谢的关系。方法:2型糖尿病(T2DM)住院患者112例,行上腹部B超检查,将其伴脂肪肝、胆囊炎、胆结石情况与脂代谢异常、T2DM病程、年龄等临床相关因素进行分析,抽样同期住院的39例非DM患者作为对照进行对比。结果:T2DM患者上腹部B超检查阳性率明显高于非DM组,T2DM患者中B超阳性组的血脂异常水平明显高于B超阴性组,亦明显高于非DM组。T2DM伴脂肪肝和(或)胆囊炎、胆结石与TC、TG、LDL-C和ApoB呈正相关,与HDL-C无显著相关性,与ApoA1、年龄和病程呈负相关。结论:T2DM患者伴脂肪肝、胆囊炎和胆结石的发病率明显高于非DM患者,其发生与脂代谢异常关系密切。

Abstract:Objective: To investigate the relationship between type 2 diabetes mellitus (T2DM)and fatty liver, cholecystitis, cholelithiasis and lipid metabolism.Methods: To collect 112 patients with type 2 diabetes mellitus who had received B-ultrasonic examination on upper belly,and analyze relative factors such age, course of disease, complication (fatty liver, cholecystitis, cholelithiasis) and lipid metabolic disorder, then contrast in parallel with 39 non-diabetes patients as control at the same time. Results:B-ultrasonic examination positive rate in diabetic patients was much higher than in negative ones. Lipid metabolic disorder in diabetes in B-ultrasonic positive group was higher than that in negative group and much higher than non-diabetic individuals.Patients with type 2 diabetes with fatty liver, cholecystitis and cholelithiasis were positive correlated to TC, TG, LDL-C and ApoB. They were negative correlated to age, course of T2DM and ApoA1. And there were none clear-cut correlation between the patients and HDL-C. Conclusion: Patients with type 2 diabetes appear more easily suffered from fatty liver, cholecystitis and cholelithiasis than inpatients with non-diabetes, which counted for partly lipid metabolic disorder.
