产前超声筛选在胎儿心脏结构异常中的应用

目的 为了避免严重心血管疾病的胎儿出生。方法对孕周达16周以上的473例孕妇进行了胎儿心脏超声筛查，其中：高危因素111例，非高危因素362例。结果从中检出心脏畸形8例，心外畸形47例，其中高危妊娠中检出心脏畸形的有7例，检出率6．3％。非高危因素中检出心脏畸形的1例。占0．28％。两组检出率有显著差异（P〈0．001）。结论多切面检查，重点放在检查胎儿四腔心切面。可以排除70％～80％的先天性心脏病。建议在高危妊娠中推广胎儿心脏多普勒超声检查。

Pregnancy Ultrasound Examine for Application in Fetal Heart Structure Abnormity

Objective To study the early fetal heart stracture malformations in order to avoid birth defect.Methods 2D doppler ultrasound examine of fetal hearts was carried out in 16 weeks of pregnant women among whom,111 were in the high risk groups,362 in the normal group.Results There were 52 cases of malformation in which 8 fetal had cardiac malgormation,47 fetal had malformation outside of the heart,and 7 fetals with congenital heart malformation were in the high risk group.The detection rate is 6.31%.Only one fetal with congenital heart malformation was in the normal group.The detection rate was 0.28%.Significantly difference was noted between two groups(P<0.001).Conclusion The congenital malformation detected in heart and outside part of heart is positively correlated to high risk fectors.Using ultrasound screening,we should emphasize examine the plane of four cavity heart in which the higher detection rate of abnormal heart structure can be increased.We suggest that fetal echocardiography as well as a prenatal fetal caidiac screening system should be undertaken for high rish pregancy.
