| 摘 要： | 目的分析细胞角蛋白19(CK19)和半乳糖凝集素3(Galectin-3)在甲状腺滤泡上皮乳头状微小病变中的表达情况。方法采用免疫组织化学染色方法检测32例甲状腺乳头状微小癌(papillary microcarcinoma of thyroid,PM-CT)(微小癌组)与28例甲状腺良性病变伴微灶滤泡上皮乳头状增生(良性病变组)CK19和Galectin-3的表达情况,所有病例术后随访1~96个月。结果 CK19和Galectin-3在微小癌组中阳性表达率分别为93.75%及96.88%,而在良性病变组表达率分别为6.25%及17.86%,两组比较差异有统计学意义(P<0.01)。随访期间,除2例PMCT失访外,余30例PMCT及28例甲状腺良性病变患者均健在,无复发或转移病例。结论 CK19和Galectin-3对鉴别诊断PMCT与甲状腺良性病变伴微灶滤泡上皮乳头状增生具有重要的临床应用价值,可作为早期PMCT的辅助病理诊断依据。PMCT预后较好,对于偶然发现或低危的PMCT可术后长期观察。
