涎腺肿瘤的彩色多普勒能量图与病理微血管密度的相关性分析

摘    要:目的 分析彩色多普勒能量图 (CDE)检测涎腺肿瘤血管的病理学基础并评价其临床应用价值。方法 对 4 5例涎腺肿瘤患者术前应用 CDE检测肿瘤的血流信号及计算机辅助测定血流信号像素网格数 ,术后对切除肿瘤组织分别应用抗血管内皮生长因子 (VEGF)、抗血管内皮细胞膜抗原 (CD34)单克隆抗体对组织进行免疫组织化学染色 ,计数肿瘤的微血管密度 (MVD)及 VEGF染色强度 ,与 CDE血流信号像素网格数作相关性研究。结果 良、恶性涎腺肿瘤 MVD与 CDE像素网格数的等级相关关系显著且均呈正相关 ,良性组和恶性组的等级相关系数分别为 0 .96 4和 0 .981。涎腺恶性肿瘤 MVD与 VEGF的表达显著高于良性肿瘤 (P<0 .0 1)且 VEGF与 MVD呈正相关。结论 病理形态学基础研究表明 CDE在诊断涎腺良恶性肿瘤中具有重要临床价值。

Abstract:Objective To study the pathologic basis of color Doppler energy(CDE) in detecting the vessels of salivary gland neoplasm.Methods A total of 45 cases with salivary gland neoplasm were examined preoperatively with CDE.The grid numbers of Doppler signal pixels which were measured with computer were assessed respectively.Neoplasm vascular endothelial growth factor (VEGF) protein and microvessels were stained immunohistochemically using anti-VEGF and anti-CD34 antibody respectively after operation.Both the microvessel density(MVD) and the intensity of VEGF staining were analyzed and the correlation between the grid numbers of Doppler signal pixels of CDE and MVD were studied.Results The grid numbers of Doppler signal pixels of CDE were showed to be positively related to MVD.The correlation coefficients of benign and malignant neoplasm were 0.964 and 0.981 respectively.The MVD and the intensity of VEGF staining in the salivary gland carcinomas were significantly stronger than those in benign salivary gland neoplasm (P<0.01) and VEGF was showed to be positively related to MVD.Conclusions The basal research of pathologico morphology expresses that CDE in distinguishing benign from malignant salivary gland neoplasm has very important application.
