海捷亚对原发性高血压患者左心室肥厚影响的研究

目的　探讨血管紧张素Ⅱ受体拮抗剂+利尿剂组合的复方制剂———海捷亚对原发性高血压患者左心室肥厚的影响。方法　入选经超声心动图检查证实为原发性高血压的左心室肥厚患者10 2例,随机分配到海捷亚组(口服1～2片/d ,每片含氯沙坦钾5. 0mg、氢氯噻嗪12 .5mg)或阿替络尔组(口服2. 5～5 .0mg/d) (n均=5. 0 ) ,治疗9个月,治疗前后各检查1次超声心动图,对比分析治疗前后左心室重量指数变化和两组间的差异。结果　①与治疗前比较,原发性高血压病患者在海捷亚或阿替络尔治疗9个月后,两组收缩压与舒张压明显下降(16 .1/ 10. 1～14 .2 / 86mmHg ;15 .9/ 10 .2～14 .3/ 90mmHg ,1mmHg =0 . 133KPa) (P均<0 . 0 1) ;②原发性高血压患者在海捷亚治疗9个月之后,左心室后壁与室间隔厚度较治疗前显著下降(P均<0 . 0 5 ) ,左心室重量及左心室重量指数下降更显著(P均<0 . 0 1) ;而在阿替络尔治疗9个月之后,左心室后壁与室间隔厚度无明显变化,左心室重量及左心室重量指数下降显著(P均<0 0 5 )。结论　原发性高血压患者在海捷亚治疗9个月后可使其左心室肥厚显著逆转,逆转作用优于阿替络尔。

COMPARATIVE EFFECTS OF HYZAAR AND ATENOLOL ON PATIENTS WITH HYPERTENSION AND LVH

Objective To investigate the effects of Hyzaar or Atenolol on the patients with hypertension and LVH. Methods 102 patients diagnosed hypertension and LVH by Intracardiac Ultrasonic Imaging were randomized to either 62.5～125 mg Hyzaar (n=56) or 25～50 mg Atenolol (n=56) for 9 months. Detecting by Intracardiac Ultrasonic Imaging before and after the trial to analyze the difference between the two groups. Results ① The hypertension of the two groups patients all have been good controlled. ② After 9 months treatment, only Hyzaar group PWTD and IVSD levels significantly decreased comparing with baseline (P<0.05); Two groups LVM and LVMI levels all significantly reduced comparing with baseline, but the Hyzaar group (P<0.01) is better than the Atenolol group (P<0.05). Conclusion The hypertensive patients with LVH treated by Hyzaar or Atenolol could significantly reduce the LVH in 9 months. The Hyzaar’s effects is better than the Atenolol’s.
