结核感染T细胞斑点试验在骨与关节结核诊断中的应用研究

目的探讨结核感染T细胞斑点试验（T—SPOT．TB）在骨与关节结核诊断中的临床应用价值。方法选取2012年3月至2013年8月骨科病区收治的疑似骨与关节结核患者317例，分别采用结核感染T细胞斑点试验（T—SPOT．TB）和结核抗体（LAM—IgG）进行诊断，比较两者对骨与关节结核诊断的临床意义（包括灵敏度、特异度、假阳性率和假阴性率指标的差别）。结果结核感染T细胞斑点试验的灵敏度和特异度明显高于结核抗体试验（P〈0．05）；同时，结核感染T细胞斑点试验的假阳性率和假阴性率明显低于结核抗体试验试验（P〈0．05）。结论结核感染T细胞斑点试验在诊断骨与关节结核中具有良好的敏感度和特异度，具有重要t临床应用价值。

The primary study of T -SPOT. TB assay in the diagnosis of osteoarticular tuberculosis.

Objective To investigate the diagnostic value of T - cell enzyme - linked immunospot assay （ T - SPOT. TB） in patients with osteoarticular tuberculosis. Methods The recorded clinical data of 317 suspected patients with osteoarticular tuberculosis during March 2012 to August 2013 ,were collected retrospeetively and reviewed systematically. By the T - SPOT. TB test and tuberculosis antibody test （LAM - IgG）. The sensitivlty, specificity, false positive rate and false negative rate in two tests was compared. Results The sensitivity and specificity of T - SPOT. TB test was significantly higher than those of tuberculosis antibody test （ P 〈 0.05 ）. The false positive rate and false negative rate of T - SPOT. TB test was significantly lower than those of tuberculosis antibody test （ P 〈 0.05 ）. Conclusion T - SPOT. TB assay possess satisfactory sensitivity and specificity in diagnosis of osteoarticular tuberculosis, and it is an effective way for the diagnosis of osteoarticular tuberculosis.
