乳腺包裹性乳头状癌的临床特点与病理组织学特征

目的探讨乳腺包裹性乳头状癌的·临床特点与病理组织学特征。方法以一乳腺包裹性乳头状癌与浸润性导管癌同时性双侧原发性乳腺癌为例，复习其病历资料、临床诊疗、病理组织学及免疫组化染色结果，并复习相关文献资料。结果患者女性，73岁，右乳外上一直径1．8cm囊性肿物。病理组织学为右乳包裹性乳头状癌，浸润性癌，膨胀性生长，周围可见导管内乳头状伴非典型增生，并见小灶状导管原位癌。免疫组化p63（-），CK5／6（-），CK8（＋）。结论包裹性乳头状癌具有独特的临床特点和膨胀性浸润等病理组织学特征。

The clinical and histopathologic features of breast encapsulated papillary carcinoma.

Objective To explore the clinical features and histopathologic characteristics of encapsulated papillary carcinoma of breast. Methods One simultaneous bilateral breast cancer with encapsulated papillary carcinoma （ EPC ） and invasive ductal carcinoma was analyzed. The indexes included the clinical data, the clinical diagnosis and treatment, histopathologic features and immumohistochemical stainings are ana- lyzed. And the related literatures are reviewed at the same time. Results The female patient is 73 years old, the intracystic mass locates at the right upper outer quadrant and the diameter of lesion is 1.8 cm. The single lesion is surrounded by a rim of fibrous connective tissue and the papil- lary architecture is lined by tall columnar epithelial cells under microscopy. Meanwhile, microscopic examination demonstrates the encapsulated papillary carcinoma accompanied with solid hyperplasia, focal ductal carcinoma in situ. Myoepithelial cell staining is negative for p63, CK5/6 at the capsule wall and periphery of tumor, meanwhile immunohistochemical staining of CK8 is positive within the tumor cells. Based on the micro- scopic examination and immunohistochemical staining, the invasive growth is defined as＂ blunt invasion. Conclusion Breast encapsulated papilla- ry carcinoma has the unique clinical features, blunt invasion and other specific histopathologic characteristics.
